and Fido are gone! One way to increase your chances of finding a lost

The New Client Form is available in two formats:
Adobe PDF (requires Adobe Reader) and MS WORD (requires MS WORD).
Select your format, click on its icon and save the document to your
hard drive. MS WORD users can open, complete, and print the form in
WORD. PDF users will need to print the form and complete it
manually. If you don't have Adobe Reader installed on your computer,
you can get it from Adobe for free. Adobe's link is at the bottom of
this page.
Please remember to...
- Bring the form with you on your first visit. If it is more
convenient, you can fax it to us. Please call our office and ask the
receptionist for our fax number.
- Plan to arrive at least 15 minutes prior to your pet's scheduled
appointment. Our staff appreciates having this time to get
acquainted with you and your pet.
- Fax or bring your pet's medical records from the breeder or your
previous veterinarian.
